Usually in Chinese you don’t say the full amount again, you just change the last numbers, otherwise the sentence would be too big in Chinese characters. A better way to phrase this would be ” The age difference between Yang Yan and the doctor was not small, they are around 21-22 years apart.” I hope this helped ^^
